Buccal fat (also called Bichat’s fat pad) is one of several encapsulated fat masses in the cheek. It is a deep fat pad located on either side of the face between the buccinator muscle and several more superficial muscles (including the masseter, the zygomatic major, and the zygomaticus minor. 

The inferior portion of the buccal fat pad is contained within the buccal space. It should not be confused with the malar fat pad, which is directly below the skin of the cheek. It should also not be confused with jowl fat pads. It is implicated in the formation of hollow cheeks and the nasal folds, but not in the formation of jowls.

What is buccal fat removal?

Buccal fat removal is a procedure performed to eliminate excess fat from the area for those who are dissatisfied with their natural fat distribution. There is a two methods to get rid of chubbiness : traditional incision method and new mini-invasive approach.

Traditional method

The buccal fat pad removal is commonly performed using local anesthesia under sleeping sedation. The typical approach involves making an incision in the mouth at the back of the oral cavity near the second upper molar. By separating the fibers of the buccinator muscle, the buccal fat becomes visible without the need for an additional incision.

Once the fat is visible, a thin casing covering the fat is opened, and the fat is gently removed while applying gentle external pressure. The base of the fat is then cauterized and removed. The remaining buccal fat pad is returned to its original position, and the oral mucosa is sutured to close the incision. The entire procedure usually takes around 30 minutes.

The pros of this procedure include enhanced facial contouring and a more defined cheekbone appearance. However, potential cons may include risks associated with surgery and the possibility of over-removal, leading to a hollowed or aged appearance.
