under-eye repositioning

Why Your Eyes May Look Tired Even When You Are Not

Under-eye fat repositioning is about much more than correcting a small pocket of fat beneath the eyes. The under-eye area may be small, but its influence on the face is anything but. Sitting almost exactly at the center of the face, this delicate area plays a surprisingly important role in how we read someone’s expression, age, and overall vitality.

When under-eye bags, bulging fat, or a pronounced tear trough create uneven shadows, the entire face can begin to look tired, hollow, or older than it actually is. You may feel completely rested, yet your face seems to tell a different story.

This is why the goal of under-eye fat repositioning is not simply to “remove the bags.” Instead, the procedure focuses on restoring a smoother transition between the lower eyelid and the upper cheek, using existing fat to soften hollows and improve the overall contour.

And despite the small area being treated, the effect can extend far beyond the lower eyelid. When the light catches the face more evenly and those persistent shadows are reduced, the eyes can look brighter, the midface more balanced, and the entire expression naturally fresher.

Why Is the Under-Eye Area So Important?

The eyes are one of the first places we look when we meet someone. Eyes one of the first areas where subtle changes in volume and contour become visible.

The skin underneath the eyes is extremely thin, and the relationship between the lower eyelid, under-eye fat and cheek creates a very delicate transition.

As this area changes, the result is not always a conventional wrinkle.  You may notice a small bulge beneath the eyes. A hollow line underneath it. Or a shadow that makeup never quite manages to cover. Or simply an expression that looks more tired than you actually feel.

Under-Eye Fat Repositioning - what treatment is it ?!

If you have searched for eye surgery internationally, you have probably come across the term blepharoplasty.

It is the familiar name for eyelid surgery.  But under-eye fat reposition does not always mean simply removing excess  fat. Under-eye fat repositioning takes a different approach.

Under-Eye Fat Redistribution Surgery Method

Instead of treating protruding fat as something that must simply be removed, the surgeon can assess whether that existing fat can be redistributed toward areas of hollowness, particularly around the tear trough.

For the right anatomy, repositioning can help create a smoother transition between the lower eyelid and cheek.

The result is not about making the under-eye completely flat. It is about reducing the bulge, softening the shadow and restoring a more continuous contour.

Why You Shouldn't Be Scared of the Transconjunctival Approach

The word transconjunctival can sound intimidating when you first hear it.  It simply refers to accessing the lower eyelid area through the conjunctiva — the inner surface of the eyelid — rather than creating an incision through the visible skin underneath the eye.

For patients who are unfamiliar with this approach, the first reaction is often:

“Wait — through the inside of my eyelid?”

But the reason surgeons use this approach is precisely because the lower eyelid can be accessed without placing a visible external incision directly beneath the eye.

At Fresh Dr. Hong Clinic, the procedure is undergoing by using a small conjunctival incision by laser for removal or redistribution of under-eye fat. We use microsurgical closure with dissolvable threads, which means there is no conventional stitch removal afterward.

Of course, any procedure around the eye requires careful surgical planning. The approach should be selected according to the individual’s anatomy and the specific problem being treated.

But the word “transconjunctival” itself should not be a reason to imagine a dramatic or frightening operation
